The requirements for a driveway permit may vary depending on your area. The road commission will likely need a certificate of liability insurance from the excavator doing the work. Zoning Permit

A permit is required when any activity within 500 feet of a lake, stream or conduit or involving 1 acre or more of land. If less than an acre or more than 500 feet from water then a waiver is required.
